Solantic now offers the ability to register online for your convenience. Please follow the steps below so that you can help us minimize your wait time!
Instructions:
- Please select your area and the center that you wish to visit.
- Click to fill out the registration form.
- Fill out all required areas of the form.
- Click Submit to email the form to the specified center prior to your visit.
- If you would like to be notified by text message when the doctor will be available to see you, please provide your cell phone number and your service provider.

Remember, you MUST bring proper identification with you to Solantic for treatment. Proper ID includes:
- Drivers License
- State issued ID
- School ID
- Military ID
- Passport
